The CUMMINS isl is a very good engine, at our shop we have had very little major engine issues before the 400,000 mile range. When you get to 400,000-500,000 it will definitely need an over haul. But to more answer your question the issues before that mileage has been water pumps, air compressors, and on a rare occasion an injector or 2.

How can I Fix my Cummins ISL 400hp engine?
Fix easy and inexpensive. In a pinch, you can buy larger hose clamps from a hardware store or other source and gang them together to get the make shift clamp around boost side of the turbo plumbing failure. Even helped a fellow heavy hauler do that one night when he did not want to bothered by a set of Kansas scales on I-70.
